Frailty Coaching Program

The Self-Management Frailty Coaching Program is a free, telephone-based program that supports adults in BC who wish to prevent and/or slow down the progression of frailty.

A trained peer frailty coach connects with you by telephone once a week for approximately 30-45 minutes for 13 weeks.

Benefits of the program

Participants will learn self-management strategies and how to follow the Canadian Frailty Network’s AVOID strategy to prevent and/or slow down progression of frailty. Key topics are: activity, nutrition, medication management, vaccinations, and social engagement.

Is this program right for me?

  • Am I feeling tired or exhausted all day?
  • Am I becoming dependent on others?
  • Am I feeling frail or concerned about becoming frail?
  • Do I want to know about the AVOID strategy to prevent and/or slow down progression of frailty?

How do I receive a peer frailty coach?

When you click on the Submit button at the bottom of the Consent Form you will be redirected to our Waitlist to complete your contact information. You will then be contacted as soon as possible to schedule an intake interview to be paired with a Frailty Coach.
