Frailty Coaching Program
The Self-Management Frailty Coaching Program is a free, telephone-based program that supports adults in BC who wish to prevent and/or slow down the progression of frailty.
A trained peer frailty coach connects with you by telephone once a week for approximately 30-45 minutes for 13 weeks.
Benefits of the program
Participants will learn self-management strategies and how to follow the Canadian Frailty Network’s AVOID strategy to prevent and/or slow down progression of frailty. Key topics are: activity, nutrition, medication management, vaccinations, and social engagement.
